Tekkers Football Academy: activity and liability agreement By creating an account and registering my child, I confirm that: • My child is fit to take part in football training and activities, and I will tell the academy of any medical condition or injury that affects this. • I understand that football is a physical activity and that some risk of injury is part of taking part. • I give permission for academy staff to give first aid and to seek emergency medical treatment for my child if I cannot be reached. • I will make sure my child follows the academy rules and the reasonable instructions of the coaches. • The academy is not responsible for loss or damage to personal belongings. • To the fullest extent allowed by law, I accept that the academy and its staff are not liable for injury, loss or damage that is not caused by their negligence. I am the parent or legal guardian of this child and I agree to the above on their behalf.
